The club or organization hosting the seminar must:

1. Send a letter on club or organization stationary confirming:
   • Date of Seminar
   • Location of Seminar
   • Format of Seminar
2. Enclose the honorium. It should be mailed to Dr. Carmen Battaglia, 335 Wexford Glen, Roswell Ga 30075
3. Pay any remaining balance in full on the day of the seminar.

Note: If the seminar is canceled the club will forfeit it's deposit.
We regret that cancellations are non-refundable.
If the cancellation is the result of a disaster, a refund of 50% of the fee paid will be refunded.
